Today, getting a patient out of the hospital and into their next place of care is slow, expensive, and mostly done by hand. Care managers spend entire days calling nursing facilities, rehab centers, and home health agencies one at a time, putting together paperwork by hand, and keeping track of everything on whiteboards and handoff notes. Nothing tells them which patient to focus on next. Each extra day a patient stays can cost the hospital thousands and keeps a bed full that another patient needs. Rondo handles the busywork of hospital discharges so care managers can get patients to their next destination faster. Over time, Rondo becomes the connection between every hospital stay and wherever the patient goes next, with nursing facilities and then insurers joining the platform as it grows. Rondo starts by pulling in information from hospital records and other sources to show care managers which patients need attention and what step will move each one closer to discharge. It then matches each patient with post-acute providers that have open beds and take the patient's insurance, so no one has to call around blindly. From there, AI agents do the legwork: they call facilities, send referrals, put together the paperwork the receiving team needs, and arrange rides, all at the same time. Care managers and clinicians still make every decision about where a patient goes but Rondo does the chasing and keeps a record of every step. As more hospitals in a market use Rondo, nursing facilities and other post-acute providers join to receive referrals and speed up their own intake. The result is faster, more reliable transitions of care, so patients get to the right next setting sooner and are set up for better outcomes.

We partnered with Tom to redesign how patients leave hospitals and every transition of care that comes after. Every day, patients who are medically ready to leave the hospital wait for a bed at a skilled nursing facility, rehab center, or home health provider. Rondo's agents handle the calls, faxes, and portal work behind discharge and placement, so care coordinators can spend their time on the decisions that matter for each patient. As more hospitals and post-acute facilities run their transitions through Rondo, it becomes the connective layer between a hospital stay and wherever the patient goes next. Tom's career bringing AI into real healthcare workflows across major health systems, payors, and biopharma companies makes him the ideal partner to bring Rondo to life. Most recently, as SVP at Vi, a leading enterprise AI platform for health, Tom helped clients across healthcare and biopharma realize the benefits of productized AI, generating more than $2B in enterprise value. Before that, at Deloitte, he focused on establishing novel agentic AI products for healthcare with interdisciplinary teams of strategists, data scientists, physicians, designers, and AI engineers. Tom's knowledge of what it takes to make AI work in production inside complex care settings sets Rondo up for success as it scales with the health systems, post-acute providers, and payors that patients depend on.

Tom before Rondo

My mom's successful battle with cancer in 2013 is what brought me into healthcare. Her diagnosis came late, from care teams that weren't working together, and it weighed heavily on our family through years of chemo and radiation. I was a chemical engineer at the time, and I pivoted into healthcare. Then and now, my personal and professional mission is to drastically improve lives using innovative healthcare solutions. As an engineer. I started out scaling lab science to manufacturing scale at P&G and J&J, then moved into building healthcare products at Deloitte, and spent time in industry as head of integration at Midwest Vision Partners, an Alpine Investors-backed eye care platform we scaled from zero-to-one. Since 2021, I've been building AI products at the leading edge of healthcare, designing and launching novel tools that support physicians, care managers, and patient service teams. At Deloitte, I led agentic AI for healthcare, launching first-of-kind products with Google and NVIDIA. I've launched production AI with three of the top five health payers, six of the top 10 biopharma companies, and three of the top 10 health systems, including 15+ agentic products taken zero-to-one into production healthcare workflows. Most recently, I served on the executive team at Vi, scaling a business into unicorn status with multiple zero-to-one agentic product launches across healthcare and life sciences. I've presented and published this work at J.P. Morgan Healthcare, HLTH, HIMSS, and Google Cloud Next, and in NEJM and the Wall Street Journal. I earned an MBA from Northwestern's Kellogg School of Management and a BS in Chemical Engineering from Cornell.

Joined Alpine Investors-backed Midwest Vision Partners as the sixth hire to build a company from zero-to-one. Partnering with physicians, we launched new patient-centric technology across the country and scaled from five to 50+ clinics.
